HCV is a member of the Flaviviridae family, and is a leading cause of chronic liver diseases and hepatocellular carcinoma. The four genotypes of HCV are the 1b, 2a, 2b and 3a. Of these, 1b and 2a are most common, and it is believed that these are the major types in Japan. These genotypes have been classified into 40 or 41 subtypes according to the amino acid sequence of the putative E2 glycoprotein. Thus, the difference of virulence is considered to be caused by the genotype. In Japan, genotype 2 is most common, and subtypes 2b and 2a are most common. Genotype 2 is also predominant in the USA. The genotype and subtype are closely related to the severity of liver diseases. Most of genotype 2 viruses show low virulence for chimpanzees, and approximately 30% of those for humans. The host factor, HLA type, and viral factor, NS5A region, have been proposed as the main factors in the mechanism of the low virulence. The amino acid sequence in the NS5A region of genotype 2 is highly conserved, but that of genotype 3 is different. In genotype 3, there are amino acid substitutions in the NS5A region. The substitution affects the function of the replicase complex, and thus may affect virulence.
